Social Services
Identifying and obtaining the health care services you need can be a challenge. But you don’t have to do it alone. Our social worker, Raquel Martin, provides emotional and supportive short-term counseling during hospitalization and works with patients, family members and caregivers to develop a plan of care that:
- Addresses concerns and potential problems after discharge from the hospital.
- Provides guidance for alternative placements such as a nursing home or assisted living facility that would best meet the needs of patients and their families.
- Recommends referrals to community agencies for meals on wheels, home health care, medical equipment, lifeline, mental health services, alcohol and other drug abuse treatment and other services.
Our medical social worker offers counseling on Medicare, Medicaid, Community Options funding or other financial reimbursement programs. She also provides information on and assistance with completing the power of attorney for health care.
