Patient and Family Services - Help Navigating the Health Care System
Identifying and obtaining the health care services you need can be challenging, but you don’t have to do it on your own. Our medical social worker, Nancy Johnson, can help identify psycho/social and economic problems and recommend solutions to ensure effective treatment and recovery. The medical social worker provides emotional and supportive short-term counseling during hospitalization and works with patients, family members and caregivers to develop a plan of care that:
- Addresses concerns and potential problems after discharge from the hospital.
- Provides guidance for alternative placements such as nursing home or an assisted living facility that would best meet the needs of patients and their families.
- Recommends referrals to community agencies for meals on wheels, home health care, medical equipment, lifeline, mental health services, alcohol and other drug abuse treatment, and other services.
The medical social worker also offers counseling on Medicare, Medicaid, Community Options (COP) funding, or other financial reimbursement programs and provides information and assistance completing the power of attorney for health care.
